Job Requirements & Qualifications :

• Report directly to the Activities Director to help with the following :
• Develops activities providing the opportunity for residents to experience sensory input - touch, smell, taste, etc., group interaction and personal achievement.
• Evaluate each resident according to his / her background, interests, abilities, physical and mental limitations and needs.

This shall serve as the base from which the individual activities program shall be developed. Provides accurate data as part of the MDS inter-disciplinary team.

• Develops appropriate records, which indicate residence attendance and participation in the program with reference to resident response to the program.
• Develops a method to implement programs within a designated budget, allocated by the facility administrator.
• Attends and participates in staff meetings, department head meetings, designated committee meetings and resident care plan conferences.

Also, shall attend mandatory staff meetings.

• Coordinates and Supervises special events, field trips and encourages residents to remain active in the community.
• Coordinates resident council meetings and ensures follow through on appropriate forms reviewed and signed by the facility administrator.
• Other duties as assigned.
• Sufficient verbal and written communication skills to perform the task required.
• Must have a valid driver’s license with clean driving record.
